NOTICE FOR APPLICANTS Documents Required by Federal Government for Employment in the United States In order to obtain a for-hire driving license through the Metropolitan St. Louis Taxicab Commission, drivers must show proof of U.S. citizenship or eligibility to work in the United States.  Drivers must attest by stating under penalty of perjury that he/she is authorized to work in the U.S. as 1) a U.S. citizen, 2) a permanent U.S. resident, or 3) an alien authorized by the Immigration and Naturalization Service (INS) for employment to a specified date. You must present to the Commission either ONE DOCUMENT FROM LIST “A” - OR - ONE DOCUMENT FROM BOTH LIST “B” AND “C”.
LIST “A” Documents that Establish Both Identity and Employment Eligibility	OR	LIST “B” Documents that Establish Identity	AND	LIST “C” Documents that Establish Employment Eligibility
·          U.S. Passport (unexpired or expired)	\	·          Driver’s license or ID card issued by a state or outlying possession of the United States provided it contains a photograph or information such as name, date of birth, sex, height, eye color, and address		·          U.S. social security card issued by the Social Security Administrations (other than a card stating it is not valid for employment) or official document issued by S.S.A. (With MTC approval)
·          Certificate of U.S. Citizenship (INS Form N-560 or N-561)	/	·          ID card issued by a federal, state, or local government agencies or entities provided it contains a photograph or information such as name, date of birth, sex, height, eye color, and address		·          Certification of Birth abroad issued by the Department of State (Form FS-545 or Form DS-1350)
·          Certificate of Naturalizations (INS Form N-550 or N-570)	\	·          School ID card with a photograph		·          Original or certified copy of birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal
·          Unexpired foreign passport, with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorizations	/	·          Voter’s registration card		·          Native American tribal document
